Bluetooth dynamic mono plays once every two times

I tried the Soundabout free app with trial pro features.

My goal was to use bluetooth dynamic mode so that my Tomtom navigation app (uses media out) plays output via bluetooth HFP. This is because my car doesn't support A2DP.

This worked when setting media output to bluetooth dynamic mono, but only every second instruction. The same issue happenned with Google Navigation and with Pandora, so it's likely not app specific.

Every time an announcement is played, the phone shows the volume bar on screen, but the sound alternates back and forth between:
- Playing audio via the car's HFP system (expected behavior)
- plays the instruction very quietly via the phone, I think it uses the low non-speakerphone calling volume (incorrect behavior)

The phone is a SGH-T999 (Samsung Galaxy S3, special edition for Tmobile USA)
The car is a 2008 Honda Odyssey
